John, each food vendor would need to complete three forms to obtain a health permit.  They need to decide whether they want to obtain a 3-day permit or an annual permit (the application is different for each one).

I have attached four forms including:

2.  Annual permit application

3.  Community Event Food Information Sheet

4.  Booth sketch

Permit fees are as follows:

3-day Minimal Risk Permit - late                $83

Annual Minimal Risk Permit - late             $229

Note:  A minimal risk permit would be for a booth serving coffee, prepackaged perishable foods, whole produce, etc.

3-day Moderate Risk Permit- late             $278

Annual Moderate Risk Permit -  late        $348

Note:  A moderate risk permit would be for a booth doing any kind of preparation with perishable food, such as scooping ice cream, preparing burritos, etc.

Per our discussion, even if someone is selling prepackaged, non-perishable food or beverages, such as bottled vitamin water or packaged almonds, they would still need to obtain a health permit.  Also, a handwashing station is required at every booth that has any open food, including sampling.
